Asset Integrity Remediation Program

Client: Major International Operator (Post-Macondo & Texas City Incidents)

Challenge

In the wake of industry-defining incidents at Texas City and Macondo, a leading operator in Trinidad & Tobago initiated a comprehensive review of its offshore and onshore assets. The audit uncovered over 3,500 deficiencies, spanning from missing documentation to serious physical risks. The client needed a structured execution strategy to prioritize, package, and resolve these issues without disrupting operations.

Solution

Avantex team members led the development and delivery of a long-term remediation program that included:
  • Prioritization Tool Development: Designed a custom framework using bow-tie methodology to assess incident barriers, rank risk levels, and sequence execution based on loss-of-containment potential.
  • Program Structuring: Consolidated all findings into an executable portfolio of over 120 discrete projects.
  • Engineering Execution: Notably delivered a complex blast wall spanning four deck levels—installed entirely without welding—to mitigate one of the most critical safety risks.
  • Mentoring & Oversight: Worked closely with local contractors to build engineering, safety, and execution capability.

Added Value

  • Executed over $500M in critical remediation projects on time and under budget.
  • Delivered all scopes without unplanned production outages.
  • Maintained a zero-incident safety record throughout the 5-year program.
  • Strengthened local contractor capabilities via sustained hands-on involvement.
